MY COMMITMENT TO YOU

All information you give me will be kept in confidence. Your personal details and medical information will not be made available to third parties unless permission to do so is granted by you. Using my judgement and experience, I will suggest certain exercises and other lifestyle advice that I believe will help you achieve your personal goals, but if at any time you have specific requests please tell me so I can accommodate them. I intend to work with you within the scope of my knowledge and competencies. Therefore when I believe it is in your best interests to see another health professional, I will refer you appropriately.

YOUR RESPONSIBILITIES TO ME

Health Screening - digital screening forms are provided

  • All clients must complete a PAR-Q before commencing any exercise programme and consultation form before a massage treatment.

  • All clients will be required to clear a COVID-19 screening as part of their consultation.

  • Your trainer/therapist may require a letter of ‘medical clearance’ from your GP. Cancellation and Lateness Policy

  • If you cancel your session/treatment with less than 24 hours’ notice or fail to show, you will be charged 100% of your session/treatment fee.

  • If the client is late for a session/treatment the session will not be extended and will end at the appointed time.

  • If the trainer is late, additional time will be added to the session/treatment or to subsequent sessions.

  • In the event that you need to reschedule or you are running late for a session/treatment, please call or text and leave a message on 07739 147987.

Online Sessions

It is the responsibility of the client to ensure:

• there is sufficient space to perform the exercises safely and move freely without obstacles or obstructions, free from furniture, equipment and other hazards – a minimum of four square metres per person

• any equipment is safe and in a suitable condition for the activity being performed

• the surrounding floor space is entirely clear to remove any hazards that may increase the risk of slips, trips or falls • there are no distractions throughout the duration of the session

• there is sufficient heating, lighting, ventilation and hydration.

As with all fitness and exercise programmes, when using this exercise video you need to use your common sense. To reduce and avoid injury, you will want to check with your doctor before beginning any fitness programme. By performing any fitness exercises without supervision, you are performing them at your own risk. SQ Active will not be responsible or liable for any injury or harm you sustain as a result of this fitness programme, online fitness video or information shared on our website or Social Media. This includes emails, videos and text.

Fee Charging Policy

Block bookings must be paid for in advance and you are advised to book all your sessions in advance to guarantee availability of the sessions you require. The client agrees to make payment before the start of the first session. Failure to make payment may result in your sessions being cancelled. All monies paid are non-refundable and cannot be cancelled for any reason.
